Medical Forms

Each scout and leader needs to have this medical form comletely filled out and signed on file (parts A and B) with the pack.  The medical form will provide us a quick reference if there are any accidents during camping trips, hikes, etc.  It must be updated every year.  The form is often required for other scouting events such as Day Camp during the summer at the fairgrounds.  NOTE:  This form is a PDF that you can type into, so you can enter your scout's information once, save it and print it when you need the form.

Tour Permits The tour permit (print on 8.5 x 14) can be found here.  This form is required for all activities and must be filed with the council office prior to each activity.
